1) Obtain a written prescription from your physician requesting physical therapy evaluation and treatment for your particular diagnosis. *Evaluations, alone, may be completed without the physician's referral, but treatment must be held until necessity is determined and prescribed by the physician.

2) If you are a returning patient with a recurrent problem or injury within the last 12 months, you may initiate physical therapy without previously obtaining a prescription.

3) If you are asymptomatic (not injured), you may begin any of the general strengthening, conditioning, or athletic training programs without physician approval.

For your convenience, these pre-admission forms are available for you to download and complete prior to your evaluation. This will speed the admission process, and avoid undesirable lapse time for paper work when you arrive at the clinic.
