The purpose of this program is to facilitate the transition from a supervised therapy program to an independent gym exercise program. The Extended Care Program (ECP) allows you to utilize your current outpatient gym facilities, allowing time for you to become more confident and independent with your program. Your therapist will designate your start and projected end date (if necessary) for the ECP. Please review the following information, and your signature will be required on a copy of this program acknowledgement. Cost - $50 per month, payable at the scheduling desk before the beginning of your program. Since Medicare and insurance companies will not pay for this type of service, the cost of the program is the responsibility of the individual. Time - Participants may use our outpatient gym during the designated days and times: Monday thru Thursday 8:00 am to 4:00 pm, and Friday 8:00 am to 12:00 pm. Gym equipment - Due to potential risks or injuries, patients are to use only the gym equipment on which they have previously received instructions from their therapists. Acknowledgement - You acknowledge that there are certain risks associated with the use of gym equipment, and you agree that Garner & Riley Physical Therapy will be held harmless for any injuries sustained during the extended care program. The therapists and current therapy patients take priority over the equipment while they're performing formal therapy treatments. We also ask that gym equipment and supplies be returned to its appropriate place after each use. Family members - Family members may accompany and assist you, if necessary. Family members including children are not allowed to utilize any equipment for personal exercise. Health Lifestyle Options - When you complete your program, and you feel comfortable with your daily exercise routine, we will provide you with several options to continue an exercise program at a local facility or specific community program.
